I certify that I am at least 18 years of age, or, if not, than I confirm that I have the consent of my parent and/or legal guardian.

 

I confirm that I have no known condition, medical or otherwise, that will interfere or preclude my participation in the transaction.

 

I understand the risks and hereby consent to the purchase and/or affixing of permanent jewelry onto my person.

 

I agree to hold harmless, and fully indemnify, LiveLoveLink, Kimberly DiRienzi, and all associates, and associated parties, and to hold such entities completely free from responsibility from any and all possible claims, damages, issues, or causes of action resulting from the transaction.
